Consultation Service Availment Process

Member presents Etiqa ID with Valid ID for verification, evaluation, & treatment. Then, Manual Fizio Medical Coordinator shall prepare the Letter of Guarantee for Consultation, which is normally pre-approved, and must be signed by the member.

If the Etiqa ID presented is expired, Medical Coordinator shall contact Etiqa for manual verification process. This may take longer time so we encourage you to secure your Letter of Guarantee prior to your appointment date.

Physical Therapy Treatment Availment Process

Members must present their Etiqa ID with valid ID and doctor's referral. Manual Fizio Medical Coordinator will then verify the presented IDs and seek approval of the treatment as indicated in the doctor's referral. Should the procedure is approved, Manual Fizio medical Coordinator will prepare the Diagnostic & Procedure Letter of Guarantee. The form must be signed by the member/patient.

Next treatment schedule shall be discussed by our Orthopedic Manual Physical Therapist, hence, securing the approved Letter of Guarantee is encouraged prior to the appointment time.

Your Etiqa ID can be used for home care services provided that the additional home care fee will be shouldered by the member/patient. Pre-approval of the treatment shall be made either by the member/patient or properly coordinated with Manual Fizio Medical Coordinator.
